Insurance Information

At John F. Nelson, DDS we make every effort to help you to maximize your insurance benefits for covered procedures.  We are Preferred Providers for many PPO insurance plans. To see if we participate with your plan, contact us at (916) 923-1696 or e-mail us using the contact box on the Home page.

Whenever possible, we gather your insurance information well ahead of your appointment time.  This allows us to research your benefits before treatment is rendered, and help you avoid unwelcome surprises with regard to your charges.

We will estimate your out-of-pocket cost, called a "Co-payment."  This amount is due at the time services are rendered.  We accept several forms of payment, inlcuding Visa, MasterCard, Discover, Debit (cards displaying a Visa or MasterCard logo), CareCredit* and cash.

Please remember that you are ultimately responsible for all charges incurred, regardless of insurance coverage.
